Forms

Form W-4 (PDF) - You may use this form to report the number of Federal exemptions you wish to claim on your paycheck.

Employee Direct Deposit Agreement (PDF) - This form will allow Certified Payroll Associates to directly deposit your employee's payroll into their bank account on payday.
